Synopsis of Family of Origin Significant Events: Describe significant events in the life cycle of the client’s family of origin.

The paper is a treatment study that includes specific client information, a theoretical case conceptualization, a clinical diagnosis and goals for treatment. It will be based on two sessions with the client that will be uploaded later on. The treatment study is to include the following elements:

Client Demographics and Current Living Arrangements: List relevant descriptive information about the client, such as age, gender, ethnicity, sexual orientation, religious affiliation, relationship status, occupation, socioeconomic class, and living arrangements.

General Appearance: Describe the client’s non-verbal presentation, such as eye contact, facial expressions, gestures, posture, style of dress, grooming and hygiene, speech patterns, habits, mannerisms, unusual physical features (e.g. disabilities, tics), and energy level.

Counseling History: Present a brief synopsis of the client’s counseling history including such information as psychiatric hospitalizations, any in-patient or out-patient services, and number of counseling sessions to date with current therapist.

Synopsis of Family of Origin Significant Events: Describe significant events in the life cycle of the client’s family of origin.

Synopsis of Current Developmental Status: Describe client’s current developmental status. Specify any recent or current stressors.

Presenting Problem: Define the presenting problem as described by the client. If applicable, describe presenting problem as presented by referral source.

Theoretical Case Conceptualization: Present the theory you are using with this client, and your theory-based hypothesis of why the current symptoms are present.

Areas for Assessment: List areas that have been assessed, or are in need of assessment.

DSM-IV-TR Multi-axial Diagnosis: Present a five axis DSM-IV-TR diagnosis. Please note if this diagnosis is provisional.

Client’s Treatment Goals: State treatment goals as specified by the client.

Therapist’s Treatment Goals: State the therapist’s goals for treatment. Specify any theory-based goals.

Concerns for Case Consultation: Clearly identify the areas in which you believe you need feedback from your instructor and peers.
