Strengths:  List the strengths of the project.  You may include: novel idea/research question, novel methodology/model system, importance of the proposed work in the field, potential for discovery and innovation etc.

Weaknesses:  What are the weaknesses of the project?  You may include background (whether it covered all pertinent information), clarity of the rationale, appropriateness of the model system, potential for knowledge creation (incremental versus transformative), explanation of anticipated results, alternative strategies, feasibility etc.

Other comments:  You can include any other comments which may help the panel review committee in considering the grant proposal.

Budget:  Comment on the budget.

Ethics/Certification:  Are there ethical issues or certification issues?
