Leadership/management. In each journal entry identify an observation made during your clinical time regarding the role of the leadership/management on your unit.

1. Clinical Date and Hours present on unit: The clinical date and hours on the unit must be identified in each journal entry. (If the student has more clinical days than submitted with required journal entries—the student will submit the clinical dates, hours and personal objectives for the remaining experiences as an ungraded journal entry. The total hours submitted must meet the required clinical hours for the course.)

2. Personal Objectives: Each entry will begin by discussing personal objectives for each clinical day and how you met the objectives. You must site specific ways you met the objectives. If you were unable to meet the objectives, what are your plans for meeting the objectives in a future clinical experience.

3. Discussion of the nursing process and clinical decision making. In each journal entry, discuss an example of how you used the nursing process to inform a clinical decision or judgment you made. Discuss an instance that you witnessed an experience nurse assist you to gain confidence in your professional decision-making skills.

4. Leadership/management. In each journal entry identify an observation made during your clinical time regarding the role of the leadership/management on your unit. This may include your preceptor, the charge nurse , nurse manager or someone else in a management role. Identify the observation and reflect on one aspect of the encounter or observation. The aspects you may want to reflect on include how the leadership/management role used power and influence, impact of the leadership/management role on the quality of patient care in the agency, or how the person in the leadership/management position is viewed as a role model. You may also reflect on any aspect about the leadership or management role.

All four journals need to follow the above format and follow with the examination of one of the 4 topic areas outlined below. Only one topic area is to be chosen per journal entry. All 4 topic areas must be completed by the end of the term but students can submit in any order. (It is suggested that the topics be investigated prior to the start of any clinical hours so application to clinical can be fostered. The examples used may come from the entire role transition clinical not only the two to three clinical dates identified in the first part of the journal entry.)
