You are required to review the case study and undertake a cause and effect diagram to help to establish the root cause(s) of the problem.

You will then concentrate on one of the root causes and undertake a 5 whys analysis.

When completing the cause & effect and the 5 whys you will need to make some assumptions about the outcomes from your investigations. You need to CLEARLY articulate what these assumptions are.

The case study includes some references to ISO 15189, you need to review these clauses and articulate what they mean for the pathology laboratory.

Introduction:
Expand on the case study and discuss which part you are going to further investigate.

Body:
Discuss the problem and the two methodologies that you have been asked to use.  a clear understanding of the fish bone diagram and the 5 whys, along with a working example for each.

When you think about the fishbone diagram,

The other part of the body is a discussion about the clauses of the standard that are invoked when something goes wrong this may include things like : how do we capture problems; what do we need to do; investigating the problem and solving it; checking to see if it is a one off issue or of greater 9systemic) concern; preventing it from occurring again; monitoring our actions; and feeding into continual improvement systems.  Note, not all of these things are relevant to any particular problem, these are purely some things to think about.   Briefly discuss what parts of the standard you would go on to use with your problem

Conclusion:
Re-iterate the root cause(s)  of the problem as you see it. Discuss how you came to this conclusion and the tools you used.  Then touch on the clauses of the standard that may be invoked when an issue is identified.
