Order Description

Introduction: Q- where did idea come from ? a. ( delete and replace with your answers) the staff in my clinic would like to understand and explore in more details the barriers to communication within our workplace. Overall aim: Q- which project have you chosen from 1-4 options page 28 from attached form( option 1: facilitation opportunities(a) group workshop face to face, option 2: team building workshop,option 3- group dynamic option 4-co-facilitation. Q- What do you propose to do in your project? Describe your intentions briefly detailing the issue you will study, the people you are involving and benefits to your workplace. Methods: Describe briefly how you plan to do this and specifically ( project time frame, facilitation methods you might use and how you might evaluate how this project performed. Literature review: what pieces of literature have you influenced project and it is design ( minimum 3 references ) ?

note:I want to outline the project proposal which is related to my work as clinical pharmacist in medical ward hospital dealing with patients and other health care team. I would like to collect data from patients during medical ward in intensive care unit to conduct survey about patient compliance to their treatment regimen to prevent any further complication that might happened later on ward in hospital. I would like to choice option 1 of facilitation because in my clinical practice, as pharmacist being a part of medical care team in hospital to provide better care plan for those patients. by survey either questionnaire or making checklist or etc.. Look at page 28 from attached form called GM51018_Week_2_Study_Guide and choice one of options that can be suit for for my project.
