An incident where a wrong red label sticker was placed on a child’s pack and given to the mother. The error was noted before leaving and promptly corrected. Apology was tendered to the mother.

Explore how this happened with a root causal analysis to look at trust policy, roles and responsibilities, accountability in nursing, etc. Also look at duty of candour and importance of being honest and open with parents.
